Medical Records Requests & Fees Hamilton Chiropractic Center provides medical records in electronic and printed formats upon request. Electronic Records: $6.50 per request. Printed Medical Records — Mississippi Fee Schedule: - First 20 pages: $20.00 - Pages 21–100: $1.00 per additional page - Pages 101 and beyond: $0.50 per additional page Additional postage charges may apply for mailed records. Upon receipt of payment, a Medical Records Release Authorization Form will be sent to the email address provided during checkout. Medical records will be processed following receipt of the completed documentation. Please allow up to three (3) business days for processing.
